This report book is super easy to use and works as a guide as well to make sure you have all the important info jotted down. I like that there is a perforation to be able to hand it to the person you are giving report to for reference. It covers some good info though looks to be more for someone who is morning shift since the times only go from 8-4 pm. I mean I suppose I could also write it in anyway. I also like that it has a rubber band thing to help keep the notebook closed for privacy purposes. I also appreciate the reference values included in the cover of the book. Allows me to stay organized during a hectic work day with several patients. Also fits right in my scrub pocket.

I work at a hospital, and they are revamping the report sheets that our PCTs/CNAs use. They want to create a universal report sheet that they all use across the hospital. The sheet they currently have out is a pilot version and are asking our PCTs for their feedback so they can improve the sheet. The current sheet is awful and even as a nurse, I don't care for it. I got this to maybe give some ideas on how to better improve the report sheet and make it something that our PCTs will like and actually use.I was surprised to see that this came in a two-pack, and that there were several pages in each book. The book is narrow, so it would fit in a scrub pocket with ease. The pages are double-sided and have enough spots for three patients on each side of the sheet. Depending on the patient load, you could remove just a couple of sheets of paper to keep in your pocket if you didn't want to carry around the whole notebook.The area to write on is kind of small, but if they made it any bigger you would have fewer patients on each page or you would have to increase the size of the notebook.The inside of the front cover has conversions and helpful information. About half of this information is not anything my PCTs will use as it is more nursing-based for IV fluid formulas, lab values, and ABG interpretations. The top half does have things my PCTs will use: a pain scale and hight and weight chart conversions.The only other thing I noticed is the columns for the vital signs are labeled 8, 12, and 4. This is great for our dayshift associates, but if you're not great at converting time to military time mentally, this could cause a challenge for my midnight staff (more so the newer team members who are getting accustomed to military time).Overall, if you are a PCT/CNA and are looking for a report sheet, I think this one is great.
