This is a three page document that you write on the first copy and it needs to transfer to the second two pages the third copy the hard copy it will not transfer to I don’t care how hard you’re write on it. It doesn’t transfer through to the third page, which is terrible because it’s the main document you need

The quality of the form is not very good, the carbon paper copying effect is not good and cannot be used normally. Moreover, the so-called FMCSA red label aluminum sticker is not sticky at all and falls off easily. The overall feeling is very impractical.

These are required annually on all commercial trucks where I live. We have an in-house mechanic which is who is supposed to fill these out. Once they review the vehicle and complete the form, there is a sticker plate that certifies that the vehicle has been inspected. These are exactly what I needed for my fleet.
