The medium for my 6 lbs dog perfectly. I like that it has the d-ring and the trim on the end. The bow comes as a separate piece that can be clipped onto the dress. Note that the sizing is different- in other brands she would be a small or extra small.

This fit my medium sized dog. It looks super small at first but can stretch out pretty big and wasn't too tight on her. She seemed to be comfortable and wore it around the house for a while. The material is soft and somewhat thin.
